Welcome The Commission welcomes public comment. To ensure that the concerns of all parties are given fair consideration at the appropriate stages of the process, it is important that decorum is maintained. When addressing the Commission, please address all statements to the Chair. Since an audio record is being made of the meeting, individuals addressing the Commission are asked to use the lectern so that the comments may be recorded. In addition, individuals addressing the Commission must provide their name and address for the record. During public hearings, an announcement will be made to open and close the public comment stages of the hearing. Any written statements or other exhibits shall be provided to the staff representative, if desired to be made a part of the meeting record. Your involvement is essential to the process and is appreciated.

5. Public Hearing: Petition No. 2004-02 (SUB) Final Subdivision, Plat Anthony Manor
Applicant: Gordon and Betty Anthony, 915 Bonita Drive, Winter Park, FL 32789.
Location: The site is contiguous on its eastern boundary to the Bank of America. The site is a vacant lot situated on the north side of Horatio Avenue to the west of the intersection of Maitland Avenue and Horatio Avenue.
Request: The applicant is requesting a final subdivision plat approval to divide an existing lot, thus creating one additional lot.

6. Decision: Petition No. 2004-02 (SPR) R.G. Therapy Services, Inc.
Applicant: Rhonda Geib, 9885 Montclair Circle, Apopka, Florida 32703
Location: The site location is 630 & 614 Maitland Avenue, Maitland, Florida 32751
Request: The applicant requests site plan review approval to convert two single-family residences into one office building. The site is located within an RS-4 zoning district.

7. Recommendation: Petition No. 2004-02 (AZ) Hollieanna Shores
Applicant: Dale McDonald, 108 Tangelo Court, Maitland, Florida (Agent for the application)
Location: Hollieanna Shores Subdivision.
Request: Amend Zoning from RS-2, residential single-family-2 to RS-3, residential single family-3 district.

Such a person must provide a method for recording the proceedings.
