Welcome The Commission welcomes public comment. To ensure that the concerns of all parties are given fair consideration at the appropriate stages of the process, it is important that decorum is maintained. When addressing the Commission, please address all statements to the Chair. Since an audio record is being made of the meeting, individuals addressing the Commission are asked to use the lectern so that the comments may be recorded. In addition, individuals addressing the Commission must provide their name and address for the record. During public hearings, an announcement will be made to open and close the public comment stages of the hearing. Any written statements or other exhibits shall be provided to the staff representative, if desired to be made a part of the meeting record. Your involvement is essential to the process and is appreciated.

5. Public Hearing: Petition No 2004-02 (AZ) Hollieanna Shores
Applicant: Dale McDonald, 108 Tangelo Court, Maitland, Florida (Agent for the application)
Location: Hollieanna Shores Subdivision.
Request: Amend Zoning from RS-2, residential single-family-2 to RS-3, residential single family-3 district

6. Public Hearing:Petition No 2004-02(SPR) R.G. Therapy Services, Inc
Applicant: Rhonda Geib, 9885 Montclair Circle, Apopka, Florida 32703
Location: The site location is 630 & 614 Maitland Avenue, Maitland, Florida 32751
Request: The applicant requests site plan review approval to convert 2 single-family residences into one office building. The site is located within an RS-4 zoning district.

7. Public Hearing: Petition No 2004-01(ALU) City of Maitland
Request: Amend Transportation Element - Policy 1.2 and Standard 1.2.2 of the 2001-2020 City of Maitland Comprehensive Development Plan to incorporate a reference to the 2002 Quality/Level of Service Handbook, State of Florida Department of Transportation 2002.

8. Public Hearing: Petition No: 2004-02(ALU) 2005-2009 Capital Improvements Program

9. Recommendation: Petition No. 2003-06 (AZ)(PD) Maitland Village Preliminary Planned Development Application
Applicant: ERM Development Corp., c/o Eli Melamed, 2973 West S.R. 434, Suite 400, Longwood, Florida 32779.
Location: Contiguous on the north and west side to Winfield Unit 1 and on the east to the City of Maitland Public Works Facility/ Fire Station/Police Sub-Station.
Request: The applicant requests approval of a Preliminary Planned Development to change the zoning classification from R-1, Single-Family Dwelling District to PD, Planned Development District for construction of 88 townhome units.

10. Recommendation: Petition No. 2004-01 (AZ)(PD) ASP, WT, L.L.C.
Applicant: City of Maitland, 1776 Independence Lane, Maitland, Florida.
Location: The property is generally north of Maitland Boulevard and Maitland Summit Boulevard and south of the Seminole County line. The site is the location of the Charles Schwab Buildings at 1950 Summit Park Drive, Orlando, Florida 32810, and a portion of the site is undeveloped.
Request: The City of Maitland, Florida proposes to rezone property annexed into the City of Maitland via Ordinance 1043 on December 9, 2002, to be consistent with Maitland development regulations. Zoning of the subject property is proposed for amendment from Orange County P-D (Planned Development District) to Maitland PD (Planned Development District).

11. Recommendation: Petition 2004-03 (AZ) Winfield Annexation Area Zoning Amendment
Applicant: City of Maitland, 1776 Independence Lane, Maitland, Florida 32751
Location: Property annexed by the City of Maitland as the Winfield Annexation Area via Ordinance 970.
Request: The administrative rezoning is necessary to transfer jurisdictional zoning requirements from Orange County to the City of Maitland.
